Billing
Our office will bill all insurance carriers. We also will bill secondary insurances. Co-payments are due at the time services are rendered. Payment is due at the time of service for all self-pay patients. Payment is due at the time of service for all services that are not covered by insurance companies. If your situation prevents you from paying your bill promptly, please contact our billing office.
Cancellations
We request at least 24 hours notice when canceling an appointment.
